Priced services and Voluntary Medical Insurance (VMI)

Dear Patients (Customers)!

To conclude the Contract for Priced Medical Services Provision, one of the following documents certifying your identity shall be available.

For the citizens of the Russian Federation:

RF passport,
RF Armed Forces Identity Card (Military Service Record Book),
Temporary Identity Card of the Citizen of the Russian Federation,
Identity Card of the Citizen of the Russian Federation who Permanently Resides Outside;
Pension Certificate;
Driving License;

For foreign citizens:

Passport of the Foreign Citizen,
Temporary Residence Permit,
Residence Permit,
Refugee Certificate,
Certificate of Petition Consideration on the Acknowledgement of Refugee Status on the Territory of the Russian Federation
Certificate of Temporary Shelter Grant on the Territory of the Russian Federation.

While concluding the agreement in behalf of a minor, it is also necessary to submit the Birth Certificate.
