Otorhinolaryngological Department

DSC_2225Otorhinolaryngological Department disposes advanced laser and endoscopic equipment, surgical microscopes, which enable the performance of low-trauma endo- and microsurgical otolaryngological operations for the diseases of nose, paranasal sinuses, nasopharyngeal cavity, pharynx, larynx and ears.

The structure of Otorhinolaryngological Department represents unique surgery unit that comprises two operating rooms, equipped with endoscopic stands, OPMI SenSera stereomicroscopes and laser surgical equipment (Sharplan 40C СО2 laser and Dornier diode semiconductor laser), which permits the use of latest diagnostic techniques and microendoscopic surgical correction of ENT organ diseases.

The majority of operations in nasal cavity are controlled by endoscope and surgical microscope; shaver is used to remove polyps from nasal cavity and paranasal sinuses. We have introduced microendolaryngeal operations with the use of СО2 laser, combined with surgical microscope, under general high-frequency anesthesia via tracheopuncture.

The department is actively involved with ear surgery as well as auditory-enhancing operations (tympanoplasty, stapedoplasty) etc.

DSC_2259The application of surgical laser in otolaryngology is indicated where conventional surgical interventions are impossible (contraindicated) or more traumatic for patient. The operating technique of laser exposure minimizes bleeding during operative and postoperative period, which represents maximally conservative impact on mucosa, thus, preserving the key functions of epithelium and lymphoid tissue and preventing rough scarring.

The specialists of ENT Department completed their training at the leading centres of microendoscopic otolaryngology in Russia, Europe and USA, and mastered laser technologies at Moscow universities.
Operations are mainly performed under general anesthesia.

Types of operative treatment performed at the department.

Operations for pharynx:

  • Adenotomy;
  • Tonsillectomy;
  • Resection of benign pharyngeal lesions (papillomas, fibromas);
  • Laser uvulotomy;
  • Uvulopalatopharyngoplasty;
  • Tonsillectomy with uvulopalatopharyngoplasty;

Operations for larynx:

  • Resection of benign neoplasms;

DSC_2207Nasal and sinus operations:

  • All the types of surgical interventions for nasal septum;
  • All the types of surgical treatment for chronic rhinitis (ultrasound disintegration, laser vaporization, vasotomy etc.);
  • Resection of benign neoplasms in nasal cavity (bleeding papillomas etc.);
  • Traditional sanitizing operation for paranasal sinuses;
  • Nasal polypotomy (conventional technique with the use of endoscope);
  • Lancing of paranasal sinus with the use of endoscopic equipment, microsinusotomy;
  • Lancing of sphenoid sinus;
  • Endoscopic polysinusotomy (sinus surgery);
  • Endoscopic dacryocystotomy.

Operations for ears:

  • Ear tract exostosectomy;
  • Middle ear shunting;
  • Major surgery for middle ear;
  • Auditory enhancing operations (surgical myringoplasty, tympanoplasty, stapedoplasty).

The department also provides surgical treatment of children aged 7 and older.

Since November 2015, the Regional diagnostic advisory centre has opened in-patient Otorhinolaryngological Department, fitted with high-technology equipment. The in-patient block of ENT Department has comfortable 1, 2 and 3-seat rooms.

Up-to-date equipment, doctor’s high proficiency, great diagnostic capabilities of the Centre – forceful laboratory, Radiology Department with SCT and MRI form the complete circle for the treatment of patients with ENT-pathology from disease diagnostics, its out-patient treatment to preoperative examination, surgical in-patient therapy and postoperative out-patient after-treatment.

Where relevant, the patient is advised by the adjacent specialists: allergologist, surdologist; gastroenterologist; neurologist; pulmonologist.

Open hours of ENT-service on Monday-Friday from 8.00 a.m. to 8.00 p.m., on Saturday from 8.00 p.m. to 3.00 p.m., off work on Sunday.
Planned admission at the Otorhinolaryngological Department takes place only after receiving otolaryngologist’s consultation at advisory reception. Admission of children under 7 years old is performed based on the decision of DC. Upon consultation with the Head of Department, the admission date and the volume of required diagnostic study are established. On the date of hospitalization, the patient at the set time relies on Admission Department for the filling of case record with his diagnostic findings.

Recurrent respiratory papillomatosis – diagnostic and management issues

Recurrent respiratory papillomatosis is characterized by the epithelization of upper respiratory tract that is mainly localized in larynx, the damage of which determines disease symptoms and severity. Papillomas represent single or papillary projections similar to cauliflower. They are usually located on vocal folds. Disease recurrence after treatment is quite frequent, though there is individual predisposition to relapses: in some cases papillomas are to be resected several times a year, in other – several years later.

Микроendoscopic excision of papillomas represents the major approach in the treatment of patients with RRP; the frequency of surgical intervention depends on disease course and symptoms.

All the surgical interventions are implemented at the Otorhinolaryngological Department of SAI “RADC” under endotracheal anesthesia in the context of direct supporting microlaryngoscopy. The sound judgment on the prevalence of pathologic process is followed by papilloma resection with the use of microsurgical instruments and и Dornier diode laser in contact mode.

The department delivers medical care within the frame of compulsory medical insurance (when indicated, to patients, residing in the countryside and in the presence of appointment issued by health care facility at the place of residence), and voluntary medical insurance as a part of fee-based services.
